For many patients in our area, access to a primary care physician remains a challenge. Without this continuity of care, patients and families often struggle to manage chronic medical conditions and may feel that their care lacks coordination. With this in mind we offer primary care through our clinic to patients over 55 without a current primary care provider. Patients seeking primary care through Blount Senior Care Partners should expect care that is individualized, accessible, and focused on maximizing function and quality of life.
Patients being cared for by our providers in the hospital or skilled nursing facility can arrange a new patient visit at time of discharge. Patients new to our practice should call our clinic to arrange an appointment.

We now offer PTNS, or percutaneous tibial nerve stimulation, for men and women with overactive bladder, urgency, and/or urge incontinence. The treatment is a gentle and gradual way to modify the nerve signals from the bladder to the brain, helping patients to go to the bathroom less and have fewer accidents.

Our nurse practitioner, Dr. Traci Brackin has specialized training and years of practice experience in foot care. We offer careful trimming and debriding of thick, long nails as well as routine foot care for the diabetic patient. Blount Senior Care Partners is trained to provide specialized care for problems of the foot and nail.

At Blount Senior Care Partners, we are able to treat the unique problems for patients with ostomies (colostomy, ileostomy, urostomy, and diversion procedures). We provide pre-operative and post-operative education, stoma site marking prior to surgery, appropriate fitting of ostomy supplies, assistance with accessories, and problems with the skin around the stoma.

Transitioning from one site of care to another poses one of the biggest challenges and risks to seniors in modern healthcare, whether its confusion about medication changes, unexpected home medical equipment needs, or changes in clinical status. Blount Senior Care Partners aims to help patients make a smooth transition home from the hospital or skilled nursing facility.
Following discharge, Blount Senior Care Partners can provide a Transition Visit, a clinic visit soon after discharge, to ensure a smooth transition back to primary care providers or to provide ongoing medical care for those without an established provider in the community.
